Loss or theft of documents issued by the Host States

In the event of the loss or theft of documents issued by the Host States, the procedure below should be followed, regardless of whether the document was issued to the official or a member of his/her family.

1. Swiss documents from the Federal Department of Foreign Affairs ("DFAE")

a) Report to the CERN Service Desk

If the loss or theft occurred on the fenced parts of the CERN sitereport it via the Service Portal or to the Service Desk (Meyrin Site, building 55, tel. 77777, open on working days from 7:30am to 6:30pm, service.desk@cern.ch)

b) Report to the Geneva police

In all cases of loss or theft (in Switzerland or elsewhere) draw up an HRT written report and send it by post to the Police cantonale genevoise, 5 Chemin de la Gravière, Case postale 236, 1211 Genève 8.

c) Issue of a new document

To obtain a new DFAE document, Physics Department Users, Cooperation Associates (COAS, except LIT) and Visiting Scientists (VISC) should apply to the Users' Office (Meyrin Site, building 61), members of contractors' staff to the Registration Service (Meyrin Site, building 55) and all others to the Cards Office (Meyrin Site, building 33, office 1-024), attaching to the application one identity photograph and the copy of the report made to the Police cantonale genevoise.

2. French documents from the Ministry of Foreign Affairs ("MAE")

a) Report to the CERN Service Desk

Cf. paragraph 1 a) above.

b) Report to the French gendarmerie

In all cases of loss or theft (in France or elsewhere), go in person either to the Thoiry or French gendarmerie nearest to your place of residence to make a report.

c) Issue of a new document

To obtain a new MAE document, Physics Department Users, Cooperation Associates (COAS, except LIT) and Visiting Scientists (VISC) should apply to the Users' Office (Meyrin Site, building 61) and all others to the Cards Office (Meyrin Site, building 33, office 1-015), attaching to the application the copy of the report made to the French gendarmerie. The MAE will issue a duplicate once only.

It is advisable to keep photocopies of the abovementioned documents in order to have all the necessary information available when reporting a loss or theft. If necessary, this information may also be obtained from the appropriate Service for an application for the issue of a new document (cf. paragraphs 1 c) and 2 c) above).

The Authorities of the Host States will not issue any new document if these procedures outlined above are not followed.
